This qualitative study explored social workers' perspectives on person-centered planning (PCP) to implement a non-standardized treatment plan for the elderly receiving long-term care services (LTC). This study explores social workers' perspectives about PCP strategies found in three theoretical characteristics- achievement goal, self-determination, and normalization process theory. Three principal research questions focus on how social workers perceive the influence of PCP strategies employing alternative methods of delivery, through shared decision making, self-determination, and common goal setting on implementing non-standardized long-term care of the elderly. The case study design was chosen to capture social workers' holistic, and meaningful characteristics of real-life events in four LTC organizations. The target population of 400 social workers in metropolitan Detroit was used to purposefully select a sample size of 26 study participants. Qualitative methods and case study design explore the distinct phenomena of social workers' perceptions of PCP strategies for long-term care of the elderly by utilizing three forms of data collection- a questionnaire, a focus group, and semi-structured interviews. Data were analyzed using inductive thematic analysis supported by the use of computer software routines, and a detailed codebook. Four themes emerged from the analysis supported by the triangulation of the data sources: The study findings revealed social workers with a path for continuous quality improvement on PCP improves the quality of care and enhance the quality of life for the elderly who receives LTC s services.
