The purpose of this project was to develop and identify funding sources and write a grant proposal to fund a program for unsheltered, chronically homeless adults who are not engaged in services. The program would be based on attachment informed care to create a safe base or safe haven for unsheltered, chronically homeless individuals to participate in services and eventually enter into long-term housing. The host agency for this project is Mental Health America of Los Angeles's Integrated Health Center, located in Long Beach, California. While researching the literature, the grant writer found that chronically homeless individuals who do not participate and engage in supportive housing programs are at risk of dying on the streets or utilizing emergency department services. If these individuals can be engaged by a care provider and a program that they trust, they will use less emergency room care, access more preventative care, and the cost to take care of them in supportive housing will be less. The California Community Foundation was selected as the most suitable funding source for this project. Submission and funding of this grant proposal was not a requirement to complete this project.
