First pregnancies are major life transitions for women, which may involve difficult physical and psychological challenges. Women who have difficulty navigating the transition may be at risk for depression during pregnancy, known as antenatal depression. Antenatal depression presents similarly to general depression, aside from the time of onset and the pregnancy-specific context. The consequences of the condition are concerning as they detrimentally affect maternal and fetal cognitive, physical, and emotional outcomes. Unfortunately, knowledge of antenatal depression is limited to the risks to mother in the postpartum period and the fetus, rather than the psychological influences that contribute to the experience of the condition. Many cases of antenatal depression go undiagnosed due to a failure amongst healthcare providers to screen for depression in the antepartum period, and the absence of depression screeners that consider the impact of sociocultural factors on the expression of depression. To expand on current knowledge about antenatal depression and to better identify the psychological, social, and cultural experiences associated with antenatal depression, five pregnant women of diverse ethnic and socioeconomic (SES) backgrounds were interviewed about their pregnancy experiences. They were also asked to complete the Edinburgh Postnatal Depression Scale (EPDS) to assess the likelihood that they would be identified as at risk for AD through the screener. EPDS scores indicated just one woman to be at risk for AD. In contrast, interview responses predicted all five women to be at risk for AD. Common themes were noted among all women that may have contributed to their risk for AD. Though these themes are not generalizable due to the small sample size, they provide helpful insights for screening and practice, as well as for the direction of future research on this topic.
