Background. Research has shown a decrease in glenohumeral impingement symptoms with therapeutic exercise when the mechanism of injury is overuse. It has also been discussed that not all patients with glenohumeral impingement have an overuse mechanism of injury. These types of patients typically do not benefit as much from therapeutic exercise intervention. Purpose. The purpose of this study was to determine if a change in sleeping position from side sleeping to supine sleeping would decrease shoulder impingement symptoms in patients with glenohumeral impingement syndrome who do not have an overuse mechanism of injury. Literature review. A comprehensive literature review was performed and showed that there was an increase in subacromial pressure in a side-lying sleeping position versus a supine position. It also showed a correlation between side of shoulder pain and side of sleeping. Case description. The patient was a 37-year-old male who presented with right shoulder pain for 18 years. On the initial evaluation he presented with 7/10 pain in his right shoulder while throwing a tennis ball. The patient slept on his back for three weeks, and each week his pain level while throwing was re-evaluated. At the final evaluation, his pain level while throwing dropped to 2/10. Discussion. In this case study, the patient was able to decrease his shoulder impingement pain by changing his sleeping position. The literature supported the use of therapeutic exercise to decrease shoulder impingement pain in patients with overuse mechanism of injury. This case study showed that therapeutic exercise intervention may not be effective for patients without an overuse mechanism of injury, and that changing a patient's sleep position may be a beneficial intervention.
