Title: Development and Evaluation of a Nurse Practitioner-Directed Mindfulness Program for Adults with Chronic Low Back Pain in an Outpatient Massage ClinicBackground: Chronic low back pain (CLBP) is the leading cause of disability globally, with more than 540 million people affected by activity-limiting low back pain at any one time (Institute for Health Metrics and Evaluation [IHME], 2018). Evidence supports the use of therapeutic massage, acceptance and commitment therapy, mindfulness meditation, and other forms of complementary alternative modalities (CAM) for the treatment of CLBP. Despite the growing attention to pain the past decade, there is a lack of consistent pain assessment and effective management, and a Worldwide Urgent Global Call to Action has resulted (IHME, 2018; National Institute of Health [NIH], 2016). CLBP involves an interplay of genetic, environmental, social, cultural, biological, and physical factors that requires a multi-modal management approach for proper assessment and treatment (Institute of Medicine [IOM], 2011a; Larner, 2014). Methods: A pretest-posttest design was utilized for this project with the aim to assist in meeting the goal of the IOM request for pain cultural transformation and the National Pain Strategy initiatives to provide patient-centered, interdisciplinary pain treatment utilizing a biopsychosocial and integrative health concept (IOM, 2011a; NIH, 2016). Interventions: In an individually owned massage clinic, eligible participants received a brief, mindfulness meditation (MM) audio intervention lasting three minutes, just prior to their scheduled therapeutic medical massage (TMM). The primary outcome was self-perceived pain intensity with secondary outcomes of pain self-efficacy improvement. Results: Measures were obtained pre-intervention, post-intervention, and at 12-weeks post-intervention using a pain numeric rating scale (NRS) as well as at baseline and 12-weeks post-intervention for the pain self-efficacy questionnaire (PSEQ). Data analysis was conducted using a paired-samples t-test to assess for changes in mean scores from baseline to post-intervention. Conclusion: This project concluded that self-perceived pain intensity in patients with CLBP after the implementation of a standardized pain assessment tool by a nurse practitioner at an outpatient massage clinic and the level of pain self-efficacy after 12-weeks post-intervention were improved as a result of the MM intervention and the implemented practice change.
