Postpartum hemorrhage is among the leading causes of maternal death in the United States and worldwide. It is not known to what extent simulation-based education impacts nurses' confidence and efficacy in early recognition and quantification of blood loss in patients with postpartum hemorrhage. The purpose of this project was to examine the relationships between simulation-based education and nurses' confidence and efficacy in early recognition and quantification of blood loss in patients with postpartum hemorrhage. The project's theoretical foundation was based on the social cognitive theory, the theory of constructivism and the adult learning theory. Clinical simulation was conducted in labor and delivery of a hospital in Northern Virginia with 31 registered nurses. Instrument consisted of 24 questions on a five point-Likert scale. A quantitative correlation predictive design was used. Survey question regarding change in practice from visual estimation to objective quantification of blood loss showed significant improvement after simulation: Means (2.2903 versus 1.2903, p < .001). Paired t-test comparisons of pre versus post group means for self-efficacy was (2.0323 versus 1.5323, P < .001), collective efficacy (1.5742 versus 1.1613, P < .001) and confidence (2.0737 versus 1.3088, P < .001). The project showed exposing nurses to simulation training significantly enhanced nurses' confidence, self and collective-efficacy in early recognition and quantification of blood loss during postpartum hemorrhage (P < .001). It is recommended that hospitals adopt simulation training as a standard educational strategy for nurses to improve nursing practice and population health.
