Purpose: This study explored the classification of dysarthria severity through quantitative methods in patients with amyotrophic lateral sclerosis (ALS).Methods: Ten speech-language pathologists (SLPs) listened to speech samples from individuals with ALS and healthy controls. They orthographically transcribed the sentences, rated speech severity (i.e. normal, mild, moderate, severe and profound) and rated listening effort and tiredness. Speaking rate and intelligible words per minute (IWPM) were also calculated. Correlations were computed between all variables to explore validity of the clinician-based severity ratings. Analysis of Variance (ANOVA) testing with post-hoc contrasts were completed to determine which measure was the best for defining severity categories and how many severity categories were needed to capture clinician perception of dysarthria severity. Pearson product correlation coefficients were also completed to explore inter- and intra-rater reliability.Results: Post-hoc analyses revealed significant differences between all five severity categories (normal, mild, moderate, severe, profound) when IWPM was used as a calculated metric of the dysarthric speech samples, along with confirmed high (p > 0.9) inter- and intra-rater reliabilities for severity rating and transcription intelligibility scores.Conclusions: IWPM was identified as the best quantitative method for defining dysarthria severity categories. Five severity categories were found to be necessary to capture SLP ratings of dysarthria severity accurately. Clinician-based severity ratings were found to be reliable and valid. The preliminary results establish a foundation for further research of evidence-based practice patterns in motor speech disorders.
