The Direct Practice Improvement (DPI) project advocated the Agency for Healthcare Research and Quality (AHRQ) recommendations of diabetes self-management education / diabetes self-care education and its wide implementation throughout the United States. Studies found that diabetes self-care education helps improve diabetes type 2 patients' health conditions. However, it was not known to what extent a brief intervention of diabetes self-care education could produce similar results. The purpose of the project was to educate diabetes type 2 patients at an adult day care facility in Miami, Florida about diabetes self-care management to improve their health conditions. Rogers' theory of diffusion of innovations provided the framework to answer the clinical questions of whether a two-week diabetes self-care education is effective in controlling and maintaining healthy blood glucose levels in patients with diabetes type 2 and changing the patients' perceptions of diabetes self-care activity. The sample comprised of 35 men and women aged from 65 to 75 years. The quantitative quasi-experimental project was structured on a pre-test, an intervention, and a post-test. Pre-and-post intervention data were collected. The IBM Statistical Software for the Social Science (SPSS) was used to analyze the data. The results showed a statistically significant decrease in the participants' blood glucose levels with a correlation of .613 and a p-value < .001. The results also indicated a significant change in the overall assessment of the participants' views of self-care activity with p-values < .001 in all subscales of the questionnaire, except for health care use that showed p = .84. The project findings may be beneficial to diabetes type 2 patients, and may provide some insights to diabetes educators, healthcare providers, and stakeholders in terms of self-care education delivery mode and cost-efficiency.
