Hospitals across the US are challenged by the Joint Commission to achieve the hospital exclusive breast milk feeding rate of 70%. Even when hospital-based strategies are implemented, evidence indicates evidence-based prenatal intervention could increase hospital rates. The purpose of this study was to test the psychometric properties of the newly developed, evidence-based, prenatal Self-Efficacy for Hospital Exclusive Breast Milk Feeding (SE-Hospital EBMF) screening tool. A cross-sectional study (N = 171) was conducted with a convenience sample of pregnant women receiving prenatal care in southwest Virginia. Construct validity was demonstrated with a confirmatory factor analysis for a respecified 4-factor model. R2 values showed high reliability for all scale items. Internal consistency was high at 0.94. Cronbach's alphas, when tested for equality, showed equal reliability for the English and Spanish version, however slightly varied across parity sub-samples. Significant difference was found between the SE-Hospital EBMF score and subsamples for the self-efficacy theory domain of physiological feedback and subsamples for prenatal intention. Logistic regression, controlling for age, education, race/ethnicity, and parity confirmed SE-Hospital EBMF socre as predictive of prenatal intention for hospital EBMF. Women with higher SE scores were 4% more likely to have prenatal intention to exclusively breast milk feed in the hospital, and women with total SE score < 95.5 (M < 8) could be at risk for suboptimal infant feeding. The prenatal SE-Hospital EBMF scale is a valid and reliable tool for quantitatively assessing the strength of self-efficacy for hospital EBMF and can be used to guide prenatal intervention and education in the study population.
