Background: Brazilian Jiu-Jitsu is a sport where two athletes compete on an unstable surface, performing movements that require static and dynamic balance when attempting to take each other to the ground. Purpose: The purpose of this study was to evaluate a Brazilian Jiu-Jitsu athlete who experiences chronic ankle sprains and instability to determine if balance training on an unstable surface is more effective than balance training on a stable surface with static and dynamic movements that require a single-limb stance. Literature review: Seven articles were reviewed and appraised. All seven articles provided evidence and support for balance training on an unstable surface. Case description: The patient was a 24-year-old male Brazilian Jiu-Jitsu athlete with a chronic history of ankle sprains and reported instability of the left lower extremity during sport-specific movements on the mat that required a single-limb stance. Outcomes: Functional outcome measures---the Star Excursion Balance Test (SEBT), Balance Error Scoring System (BESS), and the Single Limb Stance Test with eyes open and closed---were used to measure improvement in balance for movements that required static and dynamic postural control in a single-limb stance. Self-reported outcome measures were used with the Lower Extremity Functional Scale and the Foot and Ankle Disability Index. Discussion: The incorporation of unstable surfaces for balance training is essential for rehabilitation of chronic ankle instability for the Brazilian Jiu-Jitsu athlete who must perform sport-specific movements in a single-limb stance on the competition mat.
