Care coordination has emerged as a system for delivering high quality and cost-effective care to patients with complex chronic conditions. Despite widespread implementation of care coordination across the country, limited published literature exists on patient experiences of care coordination. The purpose of this pilot study was to examine the experiences of patients with multiple chronic conditions enrolled in an ambulatory care coordination program in Northern California. The study was conducted in a newly implemented care coordination program led by registered nurses and social workers. Patient surveys, administered by telephone, gathered demographic and health information, along with items selected from the Consumer Assessment of Healthcare Provider and Systems (CAHPS) survey initiative to measure patient experiences within the program. Among the 31 study participants surveyed, 87% reported a high overall rating of their care manager. Participants who screened positive for a depressive disorder (p<0.01) and participants who reported poor mental health status (p<0.05) rated their care managers less favorably. The results of this study improve our understanding of the socio- demographic and health factors associated with patient experiences of care. These findings also provide a preliminary evaluation of the care coordination program and will be used to inform future quality improvement efforts.
