A cluster randomized controlled trial was conducted in 287 elderly community dwelling Chileans (73 +/- 3 years, 62% female) after an initial screening in which those with very low serum vitamin B-12 concentrations (<120 pmol/L) were excluded for ethical reasons and treated. The trial had 3 intervention arms: 1 mg vitamin B-12 pill + 1.4 microg vitamin B-12 routine PACAM fortified formula ("pill group", n=101); placebo pill + 1mg vitamin B-12 PACAM fortified formula ("milk group", n=97); and placebo pill + 1.4 microg vitamin B-12 routine PACAM ("control group", n=89). Pills and formulas were provided daily, and participants were followed for 4 months with assessment of compliance and had a further 18 months of follow-up with no supervision. Vitamin B-12 status was defined by combining serum vitamin B-12, total plasma homocysteine, methylmalonic acid and holo transcobalamin into one parameter (combined indicator of vitamin B-12 status, cB-12) and was calculated at 4, 9 and 18 months. A linear mixed-effects model was used to analyze results. Based on cB-12, a high proportion of the screened population had low vitamin B-12 status (cB-12 < -0.5, 32.5%) at baseline. cB12 was used to demonstrate the efficacy of the intervention. The pill and milk groups had a significant (p<0.001) increase in cB12 at 4 months compared with the control group, which was reflected in a reduction of 28.6 and 27.8% low B-12 status (cB-12 < -0.5) prevalence from baseline to 0 and 9.4% (p<0.001), respectively. These low rates were maintained at 9 (1.3 and 4.9%) and 18 (4.1 and 10.4%) months, indicating the effectiveness of the intervention. The pill group had the highest cB-12 value at each follow-up compared with the milk group (p<0.001). cB-12 did not change, on average, over time in the control group. Serum folate at baseline acted as an effect modifier; the effects on cB-12 in both intervention groups were significantly (p<0.01) attenuated by higher serum folate at baseline.

A pre- and post-treatment study was conducted in the 51 elderly community dwelling Chileans (median age 73 +/- 3 years, 47% female) with low serum vitamin B-12 (<120 pmol/L) concentrations at the screening of the randomized controlled trial. We assessed the effects of a single intramuscular injection of 10 mg vitamin B-12 on vitamin B-12 status (determined by cB12) and neurophysiological function after 4 months of follow-up. Treatment produced consistent improvements in conduction in myelinated peripheral nerves; the sensory latency of both the left and right sural nerves improved, based on faster median conduction times of 3.1 and 3.0 msec vs. 3.3 and 3.4 msec, respectively (p<0.0001). A total of ten sensory potentials were newly observed in sural nerves after treatment. Participants with high serum folate at baseline (above median, ≥33.9 nmol/L) had less improvement in cB-12 (p<0.001) compared with individuals whose serum folate was below the median (<33.9 nmol/L).

Additionally, a pre- and post-treatment study was conducted in a subsample of 27out of 51 participants who were excluded from the randomized controlled trial because of having low serum vitamin B-12 at screening. This subsample was selected by confirmation of low vitamin B-12 status by at least three out of four biomarkers: serum vitamin B-12, total plasma homocysteine, methylmalonic acid and holotranscobalamin. cB12 was computed. Targeted [166 carnitines, amino acids, sugars, glycerophospholipids and sphingolipids (BIOCRATES Absolute IDQ(TM))], and untargeted [247 (109 unknown) primary metabolites (by GC-TOF-MS)] were measured at baseline and 4 months after treatment. Changes from pre- to post-treatment included increases (p<0.001) in acylcarnitines, plasmalogens and other phospholipids. Proline and one carbon metabolism moieties, i.e., methionine and cysteine were reduced (p<0.001).

Our findings indicate that vitamin B-12 provision to elderly Chileans in a vitamin B-12 fortified formula or as a pill improves vitamin B-12 status defined by cB-12, but this effect is attenuated by high serum folate. Asymptomatic Chilean elderly with poor B-12 status displayed improved conductivity in myelinated peripheral nerves after B-12 treatment and an interaction with folate status which was detected only by using cB-12.Metabolomics revealed sub-clinical effects of vitamin B-12 treatment. The metabolomic profile was characterized by increases in acylcarnitines which might reflect an improvement in mitochondrial function, and by increases in metabolic markers of myelin integrity and constituents of membrane structure such as plasmalogens and other phospholipids.
