The words of Winston Churchill, "We shape our buildings and afterwards, our buildings shape us," provided the foundational thought for this case study on the experience of the planning team for the creation of the Center for Advanced Healing at a tertiary health care center in the Mountain West. This qualitative study focused on the dynamically reciprocal shaping relationship between an organization and its built environment during a major renovation and expansion project, undertaken with the intention of transforming the patient experience.

Creating a new building is a noteworthy and disruptive occasion for an organization, expensive in terms of capital, time, and organizational attention. This phenomenon proves especially true in health care where technology-intensive buildings house a host of highly compensated professionals of varied disciples, performing high-stakes work. Health care organizations, challenged to improve patient outcomes, have begun to examine the potential for the built environment to facilitate care that is safe, effective, and patient-centered. Planning for this transformation involves a reflective process in which organizational routines and the environment that supports them can be examined and the impact of alternatives considered.

In an effort to mitigate the human tendency toward dualistic thinking, this study has been conducted from the relational, processual, and holistic perspective of the transactional worldview, first articulated by Bentley and Dewey. The theoretical framework was grounded in an integration of the work of Pragmatism, particularly G.H.Mead, and Ecological Psychology, as well as more recent scholarship consistent with these worldviews, which consider the organism-environment relationship primary to experience. Analysis of the data from document review and 22 interviews with planning team members of varied disciplines yielded findings in the areas of leadership, team experience, process for shaping the future, the organization-built environment relationship, and the impact of dualistic thinking.
