In the U.S., numbers of low birth weight (LBW) and preterm birth as well as their subsequent health problems have increased in the past ten years. Previously, the variations of maternal anxiety and the relationship to infant health status were examined in low-risk pregnancy. The findings, however, were conflicting and as such, no conclusion has been reached. In the present study, data from 165 medically high-risk pregnant women who participated in a larger study ("Nurse home care high-risk pregnant women: outcome and cost") were analyzed in order to describe the pattern of maternal anxiety during pregnancy, to examine the relationship between anxiety at each trimester and infant health status, and to identify associations between maternal characteristics and infant health status.

The Multiple Affect Adjective Checklists was used to measure anxiety once every trimester. Outcome variables included infant birth weight, gestational age, and 1- and 5-minute Apgar scores. Repeated measures ANOVA was not able to identify any variations of anxiety either in the full subsample or in the high-risk subgroups (diabetes [DM: pregestational and gestational diabetes], preterm labor [PTL: diagnosis of or at risk for preterm labor], and chronic hypertension [HTN]). Graphic portrayals, however, showed a possible pattern of anxiety over the course of pregnancy for each high-risk subgroup. Partial correlation controlling for gestation type revealed a significant negative correlation between second trimester anxiety and 5-minute Apgar score in the DM sample, but no relationships were found in the PTL and HTN groups. In the HTN sample, maternal age was the only significant predictor explaining infant birth weight. For PTL group, gestation type and parity accounted for 7.5% of the variance in gestational age, while gestation type alone accounted for 7.8% and 15.0% of the variance in 5-minute Apgar score and birth weight, respectively. Maternal characteristics were not able to predict any measure of infant health status in women with DM.

Findings suggest the need for nurses to assess high-risk pregnant women individually according to their patterns of anxiety and characteristics. The appropriate primary and secondary prevention can then be implemented to reduce problems of LBW and preterm birth.
