Alcoholism, including alcohol abuse and alcohol dependence, is a major social and public health problem at an international level. This present study explored the comorbidity of alcoholism through a cross-cultural comparison between two population-based epidemiologic studies in Taiwan and the United States. The aims of this study are: (1) to compare comorbidity patterns among persons with alcohol abuse and alcohol dependence in the United States and Taiwan; (2) to select comorbid disorders which are most strongly associated with alcoholism, and compare the sociodemographic and clinical features of alcoholism among those with comorbidity versus alcoholism alone; (3) to compare differences between the sociodemographic and clinical features of the primary alcoholics and the secondary alcoholics among those with comorbid alcoholism.Subjects--Topical Terms:

The major findings of this study were as follow: (1) Chinese in Taiwan had much lower lifetime rates of alcoholism than subjects in the United States; (2) The distributions of sociodemographic factors and clinical features of subjects with alcohol dependence only were similar to those with alcohol dependence plus abuse in both countries; (3) Antisocial personality disorder, drug abuse and dependence, manic episodes, and obsessive-compulsive disorder were the most strongly associated comorbid disorders with alcoholism in both countries; (4) Alcoholic subjects with comorbid disorders exhibited similar sociodemographic factors and clinical correlates in both countries; comorbidity was more common among females and was associated with the number of alcoholic symptoms; (5) Despite the lower rate of help-seeking in Taiwan, the patterns of specialty of help-seeking behavior were similar in both countries irrespective of alcoholic subtypes or comorbidity; medical doctors providing the major source of treatment in both countries; and (6) The sociodemographic distributions and patterns of comorbidity with alcoholism among Asians in the U.S. were similar to those in Taiwan.
