Long-term care has become a major public health issue. Growing numbers of older adults, strained family caregivers, and stretched Medicaid budgets create the need for new solutions to meet the needs and preferences of a most vulnerable population. This quasi-experimental study examines the ability of two models for home care to support Medicaid-eligible older adults in the community. It compares secondary data from Caregiver Homes, a consumer-directed model for home care that features enhanced levels of clinical oversight, to a national sample of individuals receiving services from traditional home care agencies. The analysis revealed that the Caregiver Homes model supports a frailer population, as measured by multiple variables, including greater functional dependency and more advanced age. Furthermore, Caregiver Homes serves the needs of a segment of this population that is at risk for hospitalization. Lastly, a substantial number of Caregiver Homes' clients are able to die at home. These findings suggest that Caregiver Homes represents a model for care that extends the ability of caregivers to support aging adults in the community. In this way, it is consistent with public policy initiatives that seek to reduce long-term care costs by expanding effective home and community-based services, particularly for frail individuals. The Caregiver Homes model begins to address the need to support caregivers of a high-need population. Above all, it honors a legal and moral responsibility to care for aging adults in the least restrictive setting, where individuals have the greatest opportunity to possess psychological advantages, such as autonomy and personal identity, to maintain quality of life.
