The Vision 2020 initiative proposed by the American Physical Therapy Association (APTA) encouraged autonomous physical therapy practice. Efforts to reduce costs in the health care market required physical therapists to manage limited resources to achieve patient care goals. Physical therapists have minimal exposure to business concepts in their doctorate degree programs. The purpose of this qualitative phenomenological study was to examine the experiences of new physical therapists related to business and insurance reimbursement. The study participants were practicing in private physical therapy practices less than one year after graduation. The results of the study show that physical therapists who graduate with minimal exposure to business and insurance reimbursement have minimal interest in the subjects. Second career graduates with exposure to business and insurance reimbursement had interest in learning more. New physical therapists expressed a frustrating component of patient care was complying with insurer regulations. New physical therapists also demonstrated minimal business and insurance reimbursement knowledge. New physical therapists benefited from electronic medical record (EMR) systems that automated patient billing and relieved the therapist of understanding treatment coding and billing. Instruction in leadership, marketing, management, productivity metrics, and insurance reimbursement would benefit new graduates in private practice settings. Physical therapy doctorate program coordinators should consider adding curricula that exposes physical therapy students to leadership, marketing, management, productivity metrics, and insurance reimbursement. The APTA and other continuing education providers should consider offering instruction and certification in business and insurance reimbursement.
