In face of massive internal migration in China, this dissertation brings attention to the wellbeing of the left-behind elderly in the social context that adult children, the potential out-migrants, are still the culturally expected and de facto major eldercare providers. This dissertation has attempted to untangle and explain associations between adult out-migration and the psychological and physical health of the left-behind elderly in rural China.

Drawing on China Health and Nutrition Survey (2000-2006) and China Health and Retirement Longitudinal Study (2011), this dissertation factors out migration selectivity by using propensity score analysis and fixed effects models. The results show that the migration of adult children likely has real and significant health outcomes for the left-behind elderly. However, these impacts are not universally detrimental or beneficial. Rather, various health outcomes are likely to be independently affected through different pathways, which are further stratified by familial gender roles, and moderated by the socioeconomic status of the communities where older adults resided. In addition, for some health indicators (e.g. ADLs for older females), the health outcomes are highly mediated by a series of family processes and circumstances.

Generally speaking, left-behind elderly females seem to be better off than their male counterparts. The health of left-behind older males tends to be harmed in a variety of ways, including psychological health, self-rated health, self-rated memory, and increased probability of having diabetes. Left-behind older women face increased depression symptoms, but reduced risks of cardiovascular diseases, and a possible benefit in their physical performance (ADLs). At the same time, we also see the flexibility in migration behaviors of adults (e.g., return migration) also serves as a normative protection in case of serious health conditions of the older adults.

What does rural-urban migration mean to the long-lasting rural\urban health disparity in China? My results suggest that, in contrast with the encouraging role migration plays in economic improvement for rural households, results on various indicators of physical and mental health are in general not in favor of a positive view. Instead, rural-urban adult migration, in many aspects, has widened the health disparity between the rural and urban older adults.
