The purpose of this qualitative phenomenological research study was to explore health care executive's lived experiences and perceptions regarding participation in Medicare's value-based purchasing program in acute-care hospitals. This study used a modified van Kaam method by Moustakas (1994) with in-depth, semi-structured interviews to explore the lived experiences from a purposive sample of 20 chief nurse executives in all four geographical regions in the United States (Northeast, Midwest, South, and East). Findings included the discovery of five core themes: (a) clinical process of care measures, (b) multidisciplinary teamwork, (c) reimbursement for service quality (d) education, leadership influence and development, and (e) evidence-based practice. The conclusions derived from this study suggest that efforts aimed at understanding the experiences of health care executives participating in Medicare's value-based purchasing program may lead to a greater comprehension of ways that nurse executives validate the combination of pay-for-performance financial incentives with effective changes in health care quality. Research data supports the evidence that Medicare's value-based purchasing financial reimbursement model inspires positive changes in clinical and professional practices and promotes improvements in health care quality. The study also supports the need for continued research in this area.
