Background: Between 10 - 50% of female PWID in former USSR countries, where HIV epidemics are driven by injecting drug use, are reported to exchange sex for money, drugs or goods. Research in the region shows contradictory data on the association between exchange sex and HIV for PWID, and suggests that this association might be influenced by other factors. Since female PWID who exchange sex might be a bridge population between PWID and other people, it is important to monitor their risks and suggest timely interventions.

Methods: Data come from the Integrated Bio-Behavioral Survey (IBBS) of Ukrainian PWID collected in 2011 using respondent driven sampling technique in 26 cities. Participants were interviewed about risky practices and tested with rapid HIV tests. Associations of behavioral factors with HIV status were determined using stratified logistic regression.

Results: The sample included 2,465 female PWID, who were either HIV negative (76%) or who first learned about their HIV positive status in IBBS-2011 (585, 24%). Their median age was 31 years old, 214 (8.7 %) reported having had exchange sex during the last 90 days and 190 (7.7%) used alcohol daily in the past 30 days. In the initial crude analysis no association between exchange sex and HIV was found (OR= 0.644; 95% CI 0.385-1.077), but further modeling showed that alcohol use was an effect modifier, thus analyses were performed stratified on alcohol use: compared to those who use alcohol occasionally, daily alcohol consumption and exchanging sex insignificantly increased the odds of being HIV positive (OR=1.763, 95% CI 0.905 - 4.411), while daily consumption and not exchanging sex was protective (OR=0.694, OR=0.457-0.955).

Conclusions: In the presence of injecting risks, sexual risks, such as exchange sex, may have less impact on the HIV status of female PWID. However, when alcohol is used daily, exchange sex may increase female PWID`s odds of being HIV positive. Addressing drug and alcohol dependence in female sex workers and targeted prevention efforts for female PWID who exchange sex are important for Ukrainian and other post-USSR countries` HIV prevention programs.
