Hospitalized oncology patients often receive administration of medications through central venous catheters (CVCs), which can put them at risk for life threatening bloodstream infections. Registered nurses (RNs) usually perform CVC maintenance. Their knowledge, attitudes, and self-efficacy beliefs (KASE) of CVC maintenance practices are critical to the prevention and reduction of catheter-related blood stream infections (CRBSIs) among oncology patients.

This experimental pilot study assessed the efficacy of two educational approaches: a standard and an enhanced educational intervention on the maintenance of CVCs by oncology RNs. Thirty-five RNs received the standard while 16 RNs were randomly assigned (from the total sample) to receive the enhanced education. Another subset of 16 randomly chosen RNs had their CVC maintenance practices directly observed before and after the intervention. A pre and posttest assessment of KASE was administered at baseline and one month after intervention.

Findings revealed that knowledge of oncology RNs significantly increased after an educational program (t (34) = -5.06; p < .0001). Direct observation of CVC maintenance practices showed mean dressing change scores decreased post intervention (M = 24.8, SD = 2.35 vs M = 23.3, SD = 2.84) while access scores increased (M = = 8.75, SD = 0.93 vs M = 9.25, SD = 1.24 points). The decrease in dressing change scores from pre to posttest was significant, t (15) = 2.18; p <.05. The findings of this pilot study highlight the importance of the need for further investigation to determine efficacy of educational strategies and RNs attitudes and self-efficacy beliefs towards the maintenance of CVCs.
