This study looked at whether heart rate variability (HRV) biofeedback is an effective treatment for functional gastrointestinal disorders (FGIDs) in children and adolescents. FGIDs affect approximately 10 to 25% of children in the United States. This population tends to be heavy users of medical care in a search of a treatment for the chronic pain often accompanied by symptoms of depression and/or anxiety. This study looked at whether HRV biofeedback is an effective treatment for FGIDs in children and adolescents and also tried to determine whether any changes observed in FGIDs, depression, and anxiety symptoms were due to improvement in heart rate variability. Twenty six children and adolescents completed the Screen for Child Anxiety Related Disorders (SCARED) and the Short Mood and Feelings Questionnaire (SMFQ) before and after completing HRV biofeedback training. The children and adolescents also kept a weekly pain diary. After completing HRV biofeedback training pain intensity ( p < .001) and frequency (p = .005) were significantly decreased. Depression and anxiety symptoms by both parent and child report (Child SMFQ p = .026, Parent SMFQ p < .001, Child SCARED p = .004, Parent SCARED p < .001) were also significantly decreased. Changes in heart rate variability were related to some of the changes seen in depression and anxiety symptoms. These results indicate that HRV biofeedback is an effective treatment that addresses both the pain symptoms of FGIDs and the associated symptoms of depression and anxiety.
