Introduction: In 2002, the U.S. federal government launched the National Smallpox Vaccination Program to prepare for the possibility of bioterrorism. The Laboratory Response Network (LRN) established variola testing sites across the nation. The Advisory Committee on Immunization Practices at the Centers for Disease Control and Prevention recommends that workers who handle orthopoxviruses be vaccinated every three (3) years. The purpose of this study was to examine the barriers to revaccination and the persistence of immunity, to extend the revaccination interval beyond the recommended 3 years.

Methods: Forty-five (45) laboratory workers at variola testing sites completed an online survey regarding their experiences with smallpox vaccination since 2003. Vaccine response rates were compared to a larger sample of volunteer vaccinees from the 2003 smallpox vaccination program in Nebraska. Data for the vaccine "take" rates, the interval (in years) between vaccines, and the barriers to revaccination were collected from laboratory worker's survey responses.

Results: There are numerous barriers to revaccination that range from concerns over safety, changes in medical conditions, and disruptions to home and work environments. There is no statistically significant difference in the "take" rates when compared to the interval (in years) between revaccination. Only 62% of the laboratories represented in this survey handle handle orthopoxviruses.

Conclusion: The results from this study do not support a revision to the recommended 3-year interval for smallpox vaccine revaccination, however barriers to revaccination should be considered in the recommendations. The LRN should revise the target laboratories for its revaccination policy and the ACIP recommendation for revaccination should only apply to variola testing sites that handle orthopoxviruses in the scope of their normal operation.
