Childhood obesity is an epidemic in the U.S. and results in many short- and long-term health consequences, and Latino children are particularly at risk. This dissertation research aims to advance the development of effective preventive strategies with regard to early childhood obesity. The objective of this study is to better understand the sociocultural landscape of overweight by examining how family and cultural experiences influence health behaviors and consequent childhood obesity risk among Latinos in LA County. The dissertation is structured as the 3 paper option, and has the following specific aims: a) Using mixed methods, this paper examines the influence of maternal perception on dietary intake of Mexican-American families in the WIC program; b) Using qualitative one-on-one semi-structured interview data on mothers of 3- and 4-year old WIC participants and staff, this paper seeks to unravel the complex dietary adaptation experiences of first generation and U.S. born Mexican families; c) Using mixed methods, this paper assesses the impact of the new WIC packages on the food environment of WIC families. Using 3 data sources, we specifically focused on the addition of fruits and vegetables and jarred baby foods in the food package.

Results from these studies contribute to the childhood obesity literature and expands upon existing research on Latinos. The first paper focused on maternal perception and demonstrates that maternal perception of self- and child weight status did not always match feeding behavior. Mothers may have some level of recognition of overweight in themselves and their child and thus adopted healthier feeding choices accordingly. Secondly, there are cultural considerations with regards to how Mexican mothers understand and perceive healthy weight in a child. The study of dietary acculturation in paper 2 showed that the dietary acculturation process among low-income immigrant Mexican families is complex and multidimensional, and proximal barriers often limited parents' ability to make healthy eating decisions for themselves and their families. The third paper on jarred baby foods found that the addition of fruit and vegetable jarred foods to the infant food package has been well received overall by participants. As babies get older, however, parents and caregivers want the option to choose between jarred baby foods and monetary vouchers for fruit and vegetables in order to prepare their own baby foods. Ethnic group differences suggest cultural considerations of infant feeding practices should also be considered with regards to the new WIC food package.

The overall findings of this dissertation work have important implications for how research on diets of immigrant populations are conceptualized and assessed in public health research. While much of our knowledge base in public health has been the result of extensive epidemiology and other quantitative oriented research approach, there are limitations in our understanding. Culture has been shown to affect health through shaping one's personal behavior, mediating communication between participants and health care providers, and is influenced by larger socio-political contexts. At a practical level, the mixed methods approach allows for a better understanding of the contextual environment in which many of these low-income Latino families live. These ideas may be especially important when learning from residents of communities that historically have been underserved and under-evaluated. The direction of this research, and hopefully research in nutrition and obesity in the future, will likely move toward incorporating mixed methods design since it is able to capitalize on the strengths of both approaches. As a result, we will be able to design programming and nutrition care environments that will address these barriers and move us forward in bridging the gap in health disparities.
