The possible influence of induced abortion on breast cancer risk was assessed in a cohort of 267,040 women enrolled in a randomized trial of breast self-examination in Shanghai, China. Based on answers to a baseline questionnaire, subsequent breast cancer risk was not significantly associated with ever having an induced abortion or number of induced abortions. After adjustment for potential confounders, the relative risk estimate was 1.06 (95% C.I.: 0.91, 1.25) in all women and no altered risk was observed with increasing number of induced abortions. Similar results were also observed in gravid (RR = 1.06, 95% C.I.: 0.90, 1.24) and in parous women (RR = 1.07, 955% C.I.: 0.91, 1.25).

520 $a Analysis of data from more detailed interviews of 702 cases and 744 controls yielded similar results. A possible increase in risk was observed in women who had an induced abortion after gestational week 13 in the overall analysis (for first induced abortion: OR = 1.95, 95% C.I.: 0.83, 4.56; for last induced abortion: OR = 1.46, 95% C.I.: 0.62, 3.46) and also in analyses restricted to women age <50 and women age >=50. Further analysis indicated that the possible higher risk was likely restricted to women with an induced abortion after gestational week 9 or longer before their first birth. An over seven-fold increase in risk of breast cancer was seen in women with their first or last induced abortion with gestational week 9 or longer before first birth, although the finding was not statistically significant. There was no obvious increase in risk in women with induced abortion after first birth.
520 $a In summary, the present study did not find an overall association between breast cancer risk and induced abortion, but we did observe possible increases in risk in women who had an induced abortion after gestational week 9 or later and before first birth. These observations could reflect a true increase in risk in these subgroups or they could be due to chance or recall bias. Because the procedure of induced abortion is widely performed, this possible relationship between some induced abortions and breast cancer should be the subject of further investigation.
