Heart failure is a serious and costly chronic health problem for millions of older adults in the United States. The success of treatment depends on self-care. Currently there is no well-tested instrument to measure self-care of individuals with heart failure.

The purpose of this study was to develop and perform psychometric testing of such an instrument, the Heart Failure Self-Care Inventory (HFS-CI). Orem's Self-Care Deficit Nursing Theory, specifically, the six health-deviation self-care requisites, were used as the theoretical foundation for the development and testing of the instrument. Initially, thirty items were generated from the medical and nursing literature. Content analysis indicated that the items were relevant for the measurement of self-care of individuals with heart failure.

During the study, illness narratives of seven subjects were done to determine the individual perspective of heart failure and to determine if any additional items were relevant for the HFS-CI. No new items were generated, but three themes were identified regarding self-care and heart failure as (1) Non-integration of self-care for heart failure, (2) The Health-Care Provider as self-care agent, and (3) Reduced energy, losses, and self-care.

Initially, in 50 subjects, reliability of the instrument was found to be alpha = 0.86. After revision of the instrument, the reliability of the instrument in 240 subjects was found to be alpha = 0.82.

The theoretical modeling approach indicated that the instrument measures self-care of individuals with heart failure. There were 240 subjects with heart failure in this phase of the study. Contrasted group approach indicated that the HFS-CI can discriminate between healthy adults and those with heart failure. Factor analysis indicates that the component structure of the instrument is similar in nature to the six health-deviation self-care requisites.

Conceptual and measurement issues may have affected some of the results of the validity testing. The HFS-CI may be measuring adherence rather than self-care. While adherence to the medical plan is involved in self-care, other relevant self-care actions will focus the instrument on self-care. Revising and adding items to the instrument, reconceptualizing the theoretical premises for the study, and alternative measurements methods are recommended for future research.
