Health has been included among nursing's foundational concepts since the time of Florence Nightingale. However, no dominant theory or concept of health is used by nursing in research, education, and practice. Multiple concepts of health which have been developed outside of nursing are used by nurses and theories of health developed from within are not widely used.

This study examined the presentation of health concepts in nursing text books. The purpose was to examine both the explicit definitions of health provided in nursing text books as well as the underlying assumptions about health which are included by the text book authors. An exploration of nursing roles with respect to health was included in the analysis. Literature was reviewed to determine the dominant concepts of health in use in nursing and other health disciplines.

The philosophical framework for research was one of literary analysis based on Habermas (1993) as interpreted by Ricoeur (1976; 1981). A method used by Hiraki (1992) for examination of nursing texts was adapted for this research. Six text books were selected as representative of the most popular nursing texts in six basic areas of nursing education. The general outline of each book was reviewed and selected chapters were examined in detail for concepts of health and roles of nurses in health. Both the explicit definitions of health included in the texts as well as interpretation of underlying assumptions regarding health concepts were explored.

Findings were: (1) that there is no consistency with which health is presented or defined in nursing texts; (2) that the dominant concept of health used by the texts was that of clinical health wherein health is defined as the absence of disease; and (3) the role of the nurse is presented using the problem solving nursing process framework which, while appropriate for interventions related to disease management may not be appropriate for interventions related to health. Discussion included an analysis of the political and social reasons for the findings as well as recommendations for nursing to reposition itself to more effectively to be able to conceptualize and reflect a nursing concept of health.
