The purpose of this study was to determine whether the addition of pastoral counseling to psychological counseling produces better results in reducing depression in religious, terminally-ill patients than psychological counseling alone. Pertinent literature was reviewed and the case study method was utilized because it was the most appropriate for an in-depth study of the observed experiences and depression levels of four terminally-ill cancer patients that met the criteria of being (a) terminally-ill, expected to live for less than six months, and receiving hospice care, (b) severely depressed, (c) self-identified as deeply religious, (d) lucid and communicative, (e) medicated for depression for over 1 month with minimal results, and (f) receiving psychological counseling. The patients and their caregivers received care and services from an interdisciplinary hospice team which included the social worker who provided weekly sessions of psychological counseling, and the pastoral counselor who provided weekly sessions of pastoral counseling and support for their specific religious needs. Pastoral counseling techniques incorporated observation of the patients' behavior and interactions with family members, caregivers, and other patients in the context of their cultural and religious backgrounds. Pastoral counseling treatment was provided for 12 weeks with Beck Depression Inventory (BDI) tests for depression levels at the start and finish of the intervention. Case study details, responses, expressed and measured depression levels are discussed and analyzed to determine to what extent the pastoral counseling was effective in changing patient depression levels. The data indicated that, at the beginning of the treatment, the subjects tested in the severely depressed range by their BDI scores, but at the end of the treatment, they were all less depressed. The depression levels of the four subjects were reduced by 39%, 50%, 34%, and 3% respectively. Their start-of-treatment scores averaged 38---a high depression score, and the end-of-treatment scores averaged 25---in the moderate range. The case studies and findings were discussed, analyzed, and led to the conclusion that the addition of pastoral counseling was effective in reducing the depression experienced by religious terminally-ill patients. Comparison with Psychological Counseling alone was not feasible in the absence of a control group that received only Psychological Counseling. From a multicultural population, the four subjects meeting selection criteria turned out to be Spanish speakers.Subjects--Topical Terms:
