As the new millenium dawns, the Human Genome Project will demand a redefinition of the concepts of health and illness. This new knowledge is defined as an innovation. A recurring theme in the nursing literature is the attempt to identify the barriers to diffusion and adoption of innovation. Barriers may occur at the level of the individual, the organization, the communication process, and/or the innovation.

The purpose of this study was threefold. The first purpose was to examine the effect of a diffusion vehicle upon responses to new knowledge. The second purpose was to investigate the effect of level of innovativeness of the individual nurse upon response to new knowledge. The third purpose was to investigate the effect of perceived barriers to the diffusion of an innovation upon responses to new knowledge. A post test only control group study was conducted. A random sample of nurses drawn from the memberships of the Association of Women's Health, Obstetric and Neonatal Nurses (AWHONN) and Oncology Nursing Society (ONS) participated in the study. The intervention group consisted of 90 nurses (47 AWHONN, 43 ONS) and the control group consisted by 103 nurses (58 AWHONN, 45 ONS). The American Nurses Association (ANA) booklet Managing Genetic Information: Implications for Nursing Practice was disseminated to responders assigned to the intervention group prior to completion of the survey. Respondents (n = 193) were asked to identify the genetics nursing interventions they would initiate in response to genetic case vignettes. Three control variables were also measured: barriers/enhancers to the diffusion of innovation, innovativeness of the responder, and prior response to innovation by the responder.

Nurse response to diffusion of innovation was not predicted by innovativeness, prior nurse response to innovation, barriers/enhancers to diffusion of innovation, organizational affiliation or receipt of the ANA booklet. No difference was noted between ONS and AWHONN members in the ability to improve prediction of future nurse response to diffusion of innovation by any of the variables. Innovativeness was found to characterize only a small percentage of the responders. Frequency of prior performance of genetics nursing interventions was found to be high for minimal knowledge and skill complexity and nearly absent for more complex genetics nursing interventions.
