This research has explored the predictors of use of ambulatory mental health services by Medicare beneficiaries 65 years of age and older who live in the community. Using the Medicare Current Beneficiary Study, a nationally representative research data set that links survey data and Medicare claims data together and the Andersen-Newman Behavioral Model of Service Utilization as the conceptual framework, a comprehensive examination of the relationships between various individual characteristics and the utilization of ambulatory mental health services by a nationally representative sample of individuals age 65 and older was conducted.

Females, unmarried persons, those with some college, those with Medicare and Medicaid or Medicare and supplemental insurance, those who live in urban areas, those with higher incomes, those who were previously told they had a mental disorder, those with reported poor health status and low functional status, were more likely to use ambulatory mental health services.

For social workers, these results give important information for both policymakers and practitioners. Looking at demographic trends in relation to these findings it can be projected that demand for mental health services will burgeon and that unmet need will increase unless availability, accessibility, affordability and acceptability of mental health services for elderly persons in the community is addressed.

The findings inform social work policymakers of the need to examine alternatives that will make access to care more equitable. The finding also inform direct practitioners of the need to examine better methods for outreach, identification and recognition of psychiatric problems in the elderly. Additionally, for both policymakers and practitioners, the need to explore case management models and those that integrate primary care and mental health services is highlighted.
