This study is an application of the Fishbein Theory of Behavioral Intention to the intentions of primary care physicians to refer patients to medical specialists and sub-specialists. A mail survey was conducted of primary care physicians in a southeastern state of the United States. Physicians' beliefs about possible outcomes and expectations of important others of referrals were measured in an attempt to explain behavioral intentions to refer. Structural relationships in the theory were analyzed using LISREL (Analysis of Linear Structural Relationships by the Method of Maximum Likelihood) (Joreskog & Sorborm, 1984).Subjects--Topical Terms:

The important outcomes that physicians expect when a referral is made and which were found to explain referral intentions are: (1) high quality of treatment for the patient, (2) detailed reports on a timely basis, (3) treatment for the patient at the proper monetary cost, and (4) the match of a consultant's technical ability to the patient's problem. Nine other outcome beliefs were not found to be salient in the referral intention process.

Important people or groups of people who might have expectations of a certain referral behavior and which influence the physician's referral intention are: (1) the patient, and (2) the patient's family. Five other individuals or groups of individuals were not significant in explaining the referral intention.

The health care marketing and health care management implications of the findings are discussed. In addition, recommendations of future areas of research in both the Fishbein Theory of Behavioral Intentions and physician referral behavior are suggested.
