Objective. To develop process-based quality indicators for non-pharmacological care of knee and hip osteoarthritis (OA).

Methods. The measures were designed for primary care physicians' management of OA. We reviewed clinical practice guidelines for processes of care. Proposed quality indicators were drafted in an IF-THEN-BECAUSE structure. Three practicing clinicians critically appraised the indicators. Systematic literature reviews established the strength of a process-outcome link for each indicator. The quality indicators were piloted using a patient survey dataset.

Results. Eight quality indicators were proposed: exercise (2), patient education/self-management (2), assistive devices (2), weight loss (1), physical therapy modalities (1). Systematic literature reviews revealed little research to support a process-outcome link for most of the indicators. Piloting of the indicators showed that 5%--60% of patient received these non-pharmacological treatments.

Conclusion. We have proposed eight non-pharmacological quality indicators for patients with knee or hip OA. There is little good evidence to support wide spread use of these indicators.
