Fatigue and pain during a symptom-limited maximal exercise test were examined in chronic low back pain (CLBP) patients. Thirty-four CLBP patients were classified into either the pain (n = 18) or the fatigue (n = 16) group depending on why they stopped the test.

Fundamental differences existed between the two subgroups. In the fatigue group, pain did not increase with exercise, and their RPE response was similar to that of healthy subjects. In the pain group, increased pain and excessive fatigue were noted. Excessive fatigue was correlated with the subjects' psychological status. Using RPE in addition to pain ratings with exercise is recommended to determine exercise limitations in CLBP patients.
