Infant mortality remains high in Bangladesh despite the remarkable success in the reduction of fertility in the past decade. Using Mosley and Chen's framework that illustrates various levels of determinants, this study examines how different levels of predictors interactively affect infant mortality. One question addressed in this research is whether infants born as a result of unintended pregnancies, including both mistimed and unwanted pregnancies, display higher mortality in infancy than their siblings who were intended at the time of pregnancy. While some studies examined such intra-household mortality differentials, including differences in mortality between boys and girls or among children at various birth orders, only a few have examined the effect of pregnancy intention status on infant mortality. Even less common is the use of siblings for comparison, instead of using children across households, as is the case with conventional multivariate regression models.

It is also critical for us to understand the household characteristics that predict strong links between unintended pregnancy and infant mortality for the purpose of formulating policy that targets high-risk households. While mother's education has been conceptualized as an important predictor of infant health, it could increase mortality differentials within households, as demonstrated by some past studies. This study asks whether mother's education increases the mortality disadvantage of infants who are born as a result of unintended pregnancies.

I use the 1996–97 Bangladesh Demographic and Health Survey to answer these questions. To address the problem of unobserved heterogeneity, or the differences between mothers (and households) with and without unintended pregnancies, a fixed-effects sibling comparison method is employed. With this method, mortality is compared within households, between a child born from an unintended pregnancy and his or her sibling who was wanted at the time of pregnancy.

The sibling comparison model indicates that infants born as a result of both mistimed and unwanted pregnancies display odds of dying in the first year that are seven and six times, respectively, that of the siblings born from an intended pregnancy. The finding contrasts with models using conventional random-effects models that compare infants across households. These models show no significant effect of unintended pregnancy on infant mortality.

As for the effect of mother's education on the link between unintended pregnancies and high mortality, the findings are inconclusive. When the sibling sample is stratified by mother's education, and the effect of unintended pregnancy is compared, the detrimental effect of unintended pregnancy is larger among mothers with no education than among educated mothers. However, the difference is not statistically significant. Furthermore, the result shows that high household SES as well as mother's high autonomy to go to health facilities may also reduce mortality differentials between intended and unintended pregnancies. Without controlling for these household characteristics, then, one cannot rule out the possibility that the observed, potentially beneficial effect of mother's education is confounded with these other household characteristics.
