<italic>Background</italic>. The population with type 2 diabetes frequently presents with a dyslipidemic profile consisting of hypertriacylglyceridemia, a low concentration of HDL, and a predominance of small, dense LDL particles. The preponderance of research demonstrates that omega-3 fatty acids, in the form of fish oil supplements, have beneficial effects in treating this dyslipidemia by significantly reducing the concentration of triacylglycerides (TAG) but the total serum concentration of LDL cholesterol frequently increases. If this increase in total LDL concentration results from an increase in small, dense LDL particles this change in lipid composition might place the patient at greater risk for atherosclerotic cardiovascular disease (ACVD).Subjects--Topical Terms:

<italic>Objective</italic>. The objective of this study was to determine the effects of a daily 3-gram supplementation of long-chain omega-3 fatty acids, in the form of fish oil, on serum LDL cholesterol in participants with type 2 diabetes.

<italic>Design</italic>. Twenty participants with type 2 diabetes and hypertriacylglyceridemia were randomly assigned to one of two groups. Group 1 took fish oil capsules containing 3 grams of omega-3 fatty acids for six weeks, followed by a four-week wash out before a six-week control period. Group 2 began with a six-week control period, followed by six weeks taking identical fish oil capsules and an additional four-week washout period. Blood samples were before and after the treatment period and the control period for analysis of serum lipids.

<italic>Results</italic>. There was no significant difference in total TAG concentration, VLDL-TAG, or HDL following treatment with fish oil. Following fish oil supplementation, there was a significant effect of treatment on LDL (p = 0.034) demonstrating an increase in LDL concentration. There was also a significant decrease in LDL particle size (p = 0.042) and a significant increase in LDL particle number (p = 0.027) following treatment.

<italic>Conclusion</italic>. Fish oil supplementation should be used with caution and medical supervision in this population until further research clearly identifies the effects on LDL cholesterol to avoid placing the patient at increased risk.
