The current study explored the possibility of change in the construct systems of nine psychiatric patients in the course of hospital treatment. Five of the patients had diagnoses of schizophrenia. Four of the patients had diagnoses of schizoaffective disorder. They were assessed through a version of the Kelly Repertory Grid, designed to focus on concepts of mental illness and wellness, five to ten days after admission, and twenty-five days later or at the time of discharge (whichever time came first). Constructs were elicited from patients on both occasions and used to rank elements related to the interpersonal sphere and patients' self-perceptions.

Principal components analysis was applied to each patients' grid assessment on each occasion. Spearman Rank-Correlations of the elements positioned within the construct space were calculated from the two grid occasions. The failure to find significant correlations between the elements has been regarded as an indication of significant construct system change (McCoy, 1983). In the current study, eight of the nine patients showed significant change using this measure. This proportion was shown to be significant at the.05 level by the binomial test.

Individual case studies were also presented based on analysis of the content and structure of the patients' construct systems. This analysis revealed changes in the level of correlations between constructs (Bannister' s measure of intensity), changes in the amount of variance accounted for by the first principal component of the construct system, and changes in the meaningful content of constructs and principal components.

The implications of these results were discussed with the view to using the repertory grid to assess patients' changing view of their personal worlds as they undergo psychiatric stabilization. An additional attempt to relate treatment compliance to construct change was unsuccessful. Methodological factors were discussed that may have contributed to this result.
