The overall aging of America's population, the increasing incidence of cancer or functional performance problems with age, and the combined impact of cancer, cancer treatment, and aging on functional performance in older adults during cancer treatment have worked together to bring functional performance to the interest of oncology patients, caregivers, and healthcare professionals. The purpose of this descriptive, combined quantitative and qualitative methods study was to investigate the functional performance of older adults receiving chemotherapy and to describe the experience of functional performance problems in older adults during cancer treatment.

In the quantitative component, adults ages 65 years and older with cancer (n = 50; mean age 71.6, SD 5.6) completed the Functional Performance Inventory-Short Form (FPI-SF). Participants had completed at least two months of outpatient chemotherapy for solid tumor cancer. Results describe two main areas of functional performance problems: (1) strenuous indoor or outdoor household tasks were performed with much difficulty and (2) strenuous outdoor tasks, exercise, and travel in some participants were limited by health reason. Functional performance had no age, gender, or comorbidity associations, but varied by cancer diagnosis groups. Additionally data analysis showed the FPI-SF was internally consistent and has content validity for use in older adults receiving cancer treatment.

In the qualitative component of the study, purposive sampling from the results of the FPI-SF data collection produced 9 female and 2 male participants (mean age 70.36, SD 6.64) who described their experiences in semi-structured, tape-recorded interviews. Content analysis of the interview transcripts revealed codes and led to the themes of loss and adaptation to the loss of functional performance during cancer treatment. Constant comparison was used as interviews progressed and until data saturation occurred. Findings are consistent with the literature reporting modification or replacement of activities using coping methods. The results of this study provide impetus to clinicians that, as part of providing care and treatment for older adults with cancer, it is important to attend to assessment, teaching, and individualized supportive care for functional performance changes throughout treatment.
