The rising prevalence of obesity in Hong Kong has become a public health problem since obesity is associated with significant morbidity and mortality. In response of this looming epidemic, establishing an effective weight management program is an important task.

This thesis is divided into 4 parts. The first part is an overview of obesity including the definition, prevalence, causes, consequences and treatments of obesity. The second part describes a pilot study for the effect of Orlistat, without conjunction with hypocaloric diet, on weight reduction and the efficacy of lifestyle modification on weight maintenance. The 6-month Orlistat treatment resulted significant reductions in weight, waist and hip circumferences, body fat, HbA1c, total cholesterol, LDL-cholesterol, systolic and diastolic blood pressure, as well as the improvements in quality of life. However, after the 6-month weight maintenance period, the previous reductions in weight, waist and hip circumferences, body fat and the cardiovascular risk factors significantly rebounded in the group with no further intervention, irrespective of whether they were diabetics or non-diabetics, but those in the group with further intervention could sustain their weight loss and the accompanying beneficial effects.

Based on this pilot study, 2 weight management models, a pharmacological comprehensive management program (CMP) and non-pharmacological lifestyle modification program (LMP), were designed and applied to a clinic situation. The former was group based and the duration of treatment was six months with nine visits. The latter was individual based and its duration depended on the degree of overweight and consisted of weekly visits. Both models resulted in significant reductions in body weight, body fat, waist and hip circumferences, but LMP may have lower costs. Furthermore, nearly 70% of subjects in both groups were able to maintain their previous weight loss as a result of CMP or LMP for 6 months after programs. The details of this study are described in the third part of this report.

In conclusion, the two studies of this report demonstrate that Orlistat and lifestyle modification are two effective approaches in weight management, but that without lifestyle modification, the benefits of Orlistat are not sustained on cessation of therapy.
