A diagnosis and treatment of cancer can be considered negative stressful events (Lazarus,1988) in one's life, often experienced as threatening (Spencer et al., 1999; Stanton & Snider, 1993). Longer survival from cancer means increasing numbers of individuals must adjust to this illness as a long-term chronic condition, with more complicated treatment and long-term fears of recurrence. The demands of this illness on coping skills, psychological well-being, and social support systems implies that the psychosocial adjustment of cancer sufferers is an important topic for research. Researchers have examined how patients control associated psychological stress, enhance psychosocial adjustment, and influence disease progression. There has also been an increase in the development and availability of psychosocial interventions designed to address the psychosocial needs of these patients.

The objectives of this study were to examine the effects of participation in a stress management coping skills training intervention on distress, coping and adjustment in a group of outpatients with cancer. Furthermore, the moderating effects of coping on group membership, distress and adjustment were examined. The intervention group (N = 20) was compared to a group of cancer patients who participated in an unstructured support group (N = 12). After giving informed consent, participants completed a demographic data sheet. Measures of emotional distress, coping and psychosocial adjustment were completed both before and after the intervention. The intervention consisted of 8 weekly 2-hour sessions plus weekly homework to reinforce learning.

Results indicated that after the intervention, participants in the treatment group had significantly lower scores on overall levels of distress and depression than the comparison group. However the intervention did not result in the proposed beneficial effects on coping and psychosocial adjustment.

Given the results of this study, it appears that the current intervention was effective in reducing overall distress in cancer patients. However, this study raises questions as to the appropriateness of relying on levels of emotional distress as the only indicator of psychological adjustment to cancer. Instead, psychological adjustment and distress may respond differently to treatment. Research in this area would benefit from examining psychological adjustment as a multifaceted construct.
