The purpose of the present study was to add to the evidence currently available to support the use of the Repeatable Battery for the Assessment of Neuropsychological Status (RBANS) in assessing individuals with suspected dementia.

Neuropsychological data from existing case records for 225 participants, 50 years or older, with known or suspected dementia, were used in the present study. Preliminary analyses were conducted to compute the means and standard deviations for all the measures used. The reliability estimates indicating internal consistency of Index scores were computed for the Immediate Memory (r = .90), Visuospatial/Constructional Ability ( r = .67), and Delayed Memory (r = .88) Indexes.

The second analysis utilized a method of confirmatory factor analysis to examine the pattern of covariability in the test scores in relation to the structure among RBANS subscale and Index scores that was hypothesized by the test developer. The hypothesized model was ill-defined and a second model was suggested that combined the Immediate and Delayed Memory Indexes to form a 'Memory' factor. The overall goodness-of-fit indexes, primarily, the comparative fit index (.96), the normed fit index (.94), and the first fit index (PNFI = .61 and PCFI = .62) indicated that the model represented a better fit to the data.

Cluster analysis was conducted to determine whether homogenous groups of participants could be identified on the basis of their profiles of RBANS Index scores. Six clusters of individuals were identified, with different profiles of cognitive impairment. One cluster appeared to have the least cognitive impairment, while another had the greatest impairment, and the remaining clusters fell in between, each with different patterns of relative strengths and limitations. Significant differences between clusters were found on age and education.

Finally, discriminant analysis was used to understand the relationship between RBANS Index scores and other psychological measures (selected WAIS-III subtests, Trails A & B, COWAT, and the GDS), as well as with caregivers ratings of functional limitations.

Recommendations to facilitate further research into the psychometric properties and clinical utility of the RBANS were outlined, and implications for clinicians regarding its use and interpretation were presented.
