Anger and its' manifestations represent a significant social and clinical problem in children and adolescents. It can be conceptualized as a negative, internal feeling state associated with specific cognitive and perceptual distortions, physiological changes, and action tendencies. It is a multicomponential phenomenon and various treatment programs target different aspects of anger. At present, a sufficient number of individual studies of various anger control treatments with different samples have been conducted to allow for the accumulation of data. The purpose of this study was to provide a comprehensive quantitative review of the treatment-outcome literature on cognitive-behavioral interventions for anger related problems in children and adolescents.

A total of 21 published and 19 unpublished treatment-outcome studies reported between 1974 and 1996 met the required selection criteria and were included in the meta-analysis. These studies were coded on 45 characteristics which were used as variables in consecutive data analyses. The 45 characteristics were grouped into eight categories: formal study characteristics; participant characteristics; study design characteristics; treatment techniques; formal treatment characteristics; effect sizes by domain of measurement; effects sizes by source of data; and overall effect size for study/comparison. Interrater reliability estimation indicated that this scheme provided reliable coding of the individual treatment outcome studies.

The results suggest that cognitive-behavioral therapy for anger related problems in children is in the moderate range of effectiveness, and similar to what has been found for other psychological conditions in children. Skill development and eclectic approaches seem to be more effective than affective education and cognitive problem-solving, therapies that focus on internal feeling states. There is a tendency for cognitive problem-solving treatments to be more effective for reducing the experience of anger as compared with affective education treatments. Modeling, feedback, and homework were found to be important factors in the effectiveness of therapy across all four treatment types.

Age of participants failed to relate significantly to the magnitude of effect size. Participants' gender significantly related to the magnitude of effect size, with females benefiting more than males. There was a tendency for children at the moderate range of severity of psychopathology to benefit from psychotherapy more than for children at the mild or severe levels. Analyses of the original treatment-outcome studies showed that they were likely to be short-term, group administered interventions delivered by relatively new therapists. Therefore, formal therapy characteristics and therapist experience were not significantly related to treatment outcomes and generalizations about these variables to real world clinical settings can only be made with caution.
