Rapid economic changes after Doi Moi (economic reform), increasing urban migration, and reform in health sector raise several questions for social policies. This study explores health status, the use of health services, and their determinants for different groups of migrants in comparison to non-migrants. The analysis utilizes multivariate techniques, including logistic regression, negative binomial regression, and multinomial logistic regression, and using data from the 1997--98 Vietnam Living Standard Survey and the 1997 Vietnam Migration and Health Survey. The conceptual framework developed in this study is influenced by Andersen's behavioral model of health services use, Brockerhoff's model of migration and urban child mortality, and the literature of the effects of SES on health.

The results show that, all things being equal, migrants have inferior health profile and less favorable health utilization behaviors than non-migrants. Common perceptions that migrants are extremely healthy and that migration has healthy effects are in fact due to the selectivity of migrants. There is evidence that migrants' health deteriorates faster than that of non-migrants. Furthermore, migration has a strong effect on health and the use of health services independent of the effects of socioeconomic status. The current literature that focuses on rich-poor disparities has neglected migrants because they have a higher average income than non-migrants.

Great heterogeneity among urban migrants and evidence of the "urban penalty" are observed. Residents of large cities have a higher likelihood of getting ill than rural residents, but residents of medium and small cities have the best health profile. Distinction between various groups of migrants is encouraged in future studies. Among SES variables, having access to clean water, using less polluted fuel for cooking, having more education, living in a large household, and being a male are associated with a lower likelihood of getting ill and a shorter duration of suffering from illness. Contextual analysis is advocated since community level factors show strong effects on health and the use of health services. Consideration of the temporal dimension is also recommended since the migration process and the experience of health are dynamic. Finally, policy implications are discussed.
