The ability to recognize symptoms of acute coronary syndromes (ACS) is key to minimizing morbidity and mortality. The purpose of this prospective, cohort study was to compare typical and atypical symptoms in 100 patients presenting to a Chest Pain Program. Overall, 14% met the predetermined study criteria for typical presentation. Of the 31 patients with ACS, 74.2% had atypical presentations. Patients with typical presentations were more likely to have ACS than no ACS (chi2 [1, N = 100] = 3.877, p = 0.031). Male gender, symptom location, and history of ischemic heart disease were associated with ACS. Although more men than women (76.2% vs. 70.0%), and more young than old patients (77.8% vs. 69.2%) with ACS had atypical presentations, these differences were not significant. There were no significant differences found due to symptom presentation or final diagnosis in age, BMI, ethnicity, or cardiovascular risk factors. Clinicians should not rely on classic descriptions of angina when evaluating patients with symptoms suspicious for ACS.
