Chronic pain is a major healthcare problem with significant medical, psychological and social implications. A number of theories have been developed since ancient times to explain the phenomenon of pain. The most current theory utilizes a diathesis-stress model. Research has established the efficacy of a multidisciplinary approach to treating the complex issues of chronic pain. However, this treatment is costly in terms of dollars and health care utilization.

The purpose of this study was to investigate whether certain patient factors or test scores would predict completion or noncompletion of this type of demanding, time consuming treatment. The sample population consisted of 83 (63 females and 20 males) patients who were admitted to a chronic pain management program. The patients completed an intake questionnaire, the BDI, the BAI, and the SCL-90-R prior to being admitted to the program. The mean age of the patients was 47.7, ranging from 19 to 87, with a standard deviation of 14.7. Demographic data were also collected from each patient's chart including trauma, psychiatric, alcohol and drug histories and information about physical functioning.

The results of this study indicated that none of the demographic factors were significantly correlated with program completion/noncompletion. The results also indicated a statistically significant correlation between anxiety, depression and treatment noncompletion. SCL90-R scales I-S, DEP, ANX, and PSY were also significantly correlated with treatment noncompletion. Correlations with other SCL-90-R were not significant. The results suggest that future research should focus on examining outcomes rather than pre treatment factors, to determine how to structure programs for maximum benefit to patients. In addition, more focused instruments and interviews may need to be developed to more accurately reflect psychological distress in chronic pain patients.
