This pilot study tested the efficacy of a 6-week, individual, cognitive-behavioral intervention, delivered by advanced practice nurses in the primary health care setting to adolescents with elevated depressive symptoms and their parents. The primary aim of this pilot study was to evaluate the effects of a theoretically driven reproducible intervention program with mild to moderately depressed adolescents on: (a) depressive symptoms, (b) depressogenic cognition, (c) behavior, (d) self-esteem, and (e) family communication. The secondary aims were to (a) determine the feasibility of implementing such an intervention in the primary care setting, (b) evaluate the impact of involving parents in the adolescent intervention, and (c) obtain feedback from adolescents and their parents to inform the design of a larger clinical trial to test the theoretical model and efficacy of the intervention.

Compared to adolescents in the control group, adolescents completing the CWD-A experimental intervention, had: (a) fewer depressive symptoms, (b) less suicidal ideation, (c) less depressogenic cognition, (d) fewer problematic adolescent behaviors, (e) discussed problematic behaviors with their parents with less anger, and (f) had improved family communication. In addition, adolescents in both groups improved in self-esteem, personal beliefs, and dysfunctional depressogenic attitudes post-intervention. There were moderate to large effect sizes for the CWD-A intervention on adolescent: (a) problem solving, (b) communication, (c) behavior control, and (d) intensity of conflict in discussing problematic behaviors. There were small effect sizes for the CWD-A intervention on adolescent: (a) anxiety, (b) personal beliefs, (c) affective involvement, (d) depressogenic cognition, (e) teenage role, (f) total family functioning, and (g) suicidal ideation.

Compared to parents in the control group, parents completing the CWD-P experimental intervention, had: (a) fewer depressive symptoms, (b) stronger beliefs in their ability to parent their depressed adolescent, and (c) were more affectively involved with their adolescents. In addition, parents in both groups rated their teens as exhibiting fewer problematic behaviors and as having less intense discussions about these behaviors post-intervention.
