The purpose of this study was to develop an understanding of the effects of trust on individuals' health behaviors within managed mental health care systems. The health care delivery system has rapidly changed from the traditional fee-for-services system to the managed care system in response to the high rising costs of health care. However, many management strategies used by the managed care organizations have raised concerns including the integrity of the trust relationship between professionals and patients. Of special concern is the association between this trust relationship and health care outcomes as managed care organizations become more actively involve in clinical decision making.

In addition, due to the complexity of mental health services, the managed care organizations have often carved out the mental health services by subcontracting with the behavioral health care organizations (BHOs). Many individuals with severe mental illness (SMI) have comorbid health conditions, further complicating their health care needs. The changes occurring in the health care delivery system may result in complicated and fragmented health care delivery to individuals with SMI. Therefore, it is important to understand the trust relationship between the health care professionals and individuals with SMI in the current health care environment in order to further examine the effect of this trust relationship on health care outcomes.

The study explored the relationships between trust in health care providers and selective health outcomes through a population based mail survey. The results indicated that individuals with a higher level of trust in their health care providers tended to be more satisfied with health services received and were more adherent to treatment. They were also less likely to use the more expensive emergency services. In addition, individuals with severe mental illnesses trust in their mental health care providers more than in their physical health care providers.

These findings provide health professionals, administrators, and health policymakers a further understanding about the impact of trust on health care outcomes and possibly health care costs in managed care environments.
