Introduction. There is evidence that work related musculoskeletal disorders (WMSDs) have a significant impact on physical therapists (PTs). In prior studies, PTs have reported taking sick time, changing practice habits, changing work settings or leaving the profession due to WMSDs. There are very few studies that have adequately evaluated the relationship between PT exposures and WMSDs however. Methods.{09}This was a prospective cohort study with a one year follow up. Data were collected with two postal questionnaires, one at baseline and one a year later. Data on background and demographic factors, risk factors and musculoskeletal outcomes were included in the questionnaires. Annual prevalence and incidence rates of WMSDs were calculated. A case was defined as a work related musculoskeletal disorder rated at least 4/10 on a visual analogue pain scale and lasting at least a week or present at least once a month. The relationship between risk factors and outcomes were evaluated with multivariate logistic regression.{09}Both the cross sectional and longitudinal data were analyzed.

Results. The annual prevalence rates were 34.1% (baseline) and 28.0% (follow up). The annual incident rate at follow up was 20.7%. The most common sites of WMSDs were the low back, the wrist and hands, and the neck. Therapists reported losing work time due to WMSDs (7.0%), seeing a physician because of WMSDs (13.0%), changing settings because of WMSDs (2.0%) and/or leaving the profession due to WMSDs (0.5%).{09}Risk factors that consistently emerged during both analyses included increasing age, awkward postures, soft tissue work and patient handling tasks.

Conclusions. About 1/3 of therapists reported WMSDs and about 1/5 of therapists developed a new case over one year. Age, soft tissue work and patient handling were consistently associated with WMSDs. More attention to this problem is needed within the profession.
