Distraction Osteogenesis is a process of bone regeneration under tensile stress with axial rhythmic distraction after osteotomy. Bone maturation (collagen alignment and matrix mineralization) is a very slow process. Recently, therapeutic low intensity pulsed ultrasound (LIPUS) had been reported to shorten fracture healing time. It has also been tested to enhance bone maturation in distraction osteogenesis but it was still controversial. Thus we hypothesize that LIPUS may facilitate the bone formation in different stages of distraction osteogenesis and positive effects of LIPUS during fast distraction is dose-dependent.

Open osteotomised rabbit tibia was distracted with 1mm/day for 2 weeks as normal distraction at 7-day post-operation. LIPUS was applied during distraction, latent-distraction, consolidation and fast distraction period (2mm/day, 1 week). Plain X-ray, DXA and pQCT assessed the bone mineral changes in distraction callus. Furthermore, histomorphometrical analysis evaluated cell-matrix relationship.

Results showed that LIPUS induced bone regeneration of distraction callus with spatially different stimulation. The proximal and distal regions of distraction gap were more susceptible to LIPUS stimulation. In histomorphometry analysis, LIPUS not only stimulated the intramembranous bone formation, but also chondrogenesis at the initial period of distraction. Subsequently more osseous tissues were formed in the late period of distraction. It might be due to endochondral ossification. In fast distraction study, both bone and cartilage formation was enhanced by LIPUS in a dose-dependent manner as shown in bone histomorphometry.

In consolidation stage, LIPUS augmented bone mineralization at the initial period and then remodeling. The mechanical properties of LIPUS treated distraction callus might reach its peak at the early period of consolidation, significantly greater than that of controls, as reflected by bone strength index. However, such difference was disappeared at late period of consolidation between two groups.

In conclusion, LIPUS was an effective treatment method to enhance bone regeneration during distraction and consolidation stage of distraction osteogenesis. LIPUS also augmented bone formation in fast distraction and showed dose-dependent effect. Results of the present study might provide insights in the application of LIPUS treatment on clinical limb lengthening.
