The issue of medical comorbidity among individuals with serious mental illness is still an area of research that has received little attention in the literature. Very few studies investigate the factors related to better health outcomes among individuals diagnosed with serious mental illness. Studies have shown that individuals comorbidly diagnosed with a serious mental illness and a physical illness are at a particularly higher risk of premature death as compared to individuals with only a physical illness. It is unclear what factors contribute to this overall increased mortality risk and the overall poor physical health outcomes of individuals with psychiatric illnesses.

The present study examined the relationship between intraindividual variables and health related outcomes among people dually diagnosed with diabetes and schizophrenia. The study investigated the role of illness cognitions, a sense of optimism, health locus of control, and coping styles in predicting a range of health behaviors/outcomes including overall health status as well as diabetes specific health status.

Some of the hierarchical multiple regression analyses showed significant results. The independent variables were significant predictors on the general health status measure (i.e., General Physical Health Composite Score). As a set, optimism and the internal health locus of control variable, was found to be significant only with the general health status measure (i.e., Physical Health Composite score of the SF-36). The first set did not significantly predict scores on SDSCA Diet, SDSCA Exercise, SDSCA Glucose, and GHb outcome measures. More specifically, optimism, internal health locus of control, and the IMIQ Controllability subscale were significant predictors of the General Physical Health Composite score of the SF-36) but not significant predictors for the SDSCA Diet, SDSCA Exercise, SDSCA Glucose, and the GHb outcome measures.

The second set, comprised of the Illness Cognitions (i.e., IMIQ Seriousness, IMIQ Personal Responsibility, IMIQ Controllability, and IMIQ Changeability) did not significantly contribute to the prediction of the general health status measure, SDSCA Diet, SDSCA Exercise, SDSCA Glucose, and GHb scores. The third set, Coping set, did not significantly predict scores on any outcome measure (i.e., the general health status measure, SDSCA Diet, SDSCA Exercise, SDSCA Glucose, and GHb).
