This is a meta-analysis of aerobic and non-aerobic exercise as treatments for symptoms of anxiety and depression. The introduction includes a review of the following topics; the comorbidity of anxiety and depression, the assessment of anxiety and depression and the theoretical and empirical rationale for the exercise intervention, and a description of the results of previous meta-analyses of exercise interventions as well as the more conventional treatments for anxiety and depression which include psychotherapy and pharmacotherapy. Studies were included in the present meta-analysis if they utilized aerobic or non-aerobic exercise for the treatment of symptoms of anxiety or depression in persons who were experiencing elevated levels of anxiety, stress or depression.

The other major inclusion criterion was the use of a relevant comparison group, which could include; a no-treatment or minimal treatment control group, psychotherapy, pharmacotherapy, or an alternative form of exercise (aerobic vs. non-aerobic). Effect sizes for individual studies were calculated using the formulae recommended by Hedges and Olkin (1985). Significant differences in the effect sizes between groups of studies on a number of variables of interest were determined though the use of Q tests of homogeneity which are analogous to an analysis of variance. Differences between the effect sizes for various classes of comparison groups were analyzed, including, waitlist or no treatment control; psychotherapy and exercise plus psychotherapy; pharmacotherapy; social activities, leisure activities, or occupational therapy. There was also an analysis of comparisons of aerobic vs. non-aerobic exercise and low intensity vs. high intensity exercise.

Findings of this meta-analysis support and extend the findings of previous analyses. Exercise can produce large effect sizes in the reduction of anxiety and depression in persons with mild to moderate levels of distress. A larger effect size was obtained in the reduction of anxiety than in previous analyses. Effect sizes were similar for participant groups with psychiatric diagnoses and without diagnoses. However, effect size magnitude is strongly affected by the type of comparison group which is used, with comparison with waitlist controls resulting in the largest effect sizes. Attrition data demonstrate that exercise is well tolerated, with low intensity exercise, being better tolerated than high intensity exercise.
